prospective Members of CHC including:
1.) The completion of
Confidential Health Questionnaires;
2.) The completion of a
Checklist and Acknowledgment; and
3.) The execution of a
Business Association Contract
(The original June 5th deadline has been extended)

be completed & returned
by June 19, 2009 to ensure
coverage
this year.
